INJURY LIABILITY WAIVER
No player will be allowed to participate in any act ivity organized by La Liga Futbol Camps, Inc. training and events without the Waiver of Liability and Release Form signed, properly executed, and on file. Player ́s status will be kept in good standing. Pla yers will not compromise themselfs in such a way as to do harm to La Liga Futbol Camps, Inc , kn owing that players may be dismissed from participation, with possible loss of payment or due s, for violent conduct or unsportsmanlike behavior on or off the field of play. Parents/legal responsible person of the players agree to pay for any and all damages to any property or inde mnities caused by the player willfully, negligently, or otherwise.
Soccer is a physical, contact, sport that involves the risk of injury. Players assume all risks and hazards associated with my participation in the spo rt. I am in proper physical condition to participate in soccer practices and games and have no illness, disease or existing injury or physical defect that would be aggravated by my part icipation. Players will inform their coach if this status changes. Players further acknowledge th at this risk may involve loss or damage to me or my property, including the risk of death, or other unforeseen consequences, including those which may be due to the unavailability of imm ediate emergency medical care. Players have a current medical consent form in force. Playe rs will wear appropriate soccer gear, including shin guards and protective gear, when req uired.
La Liga Futbol Camps, Inc. have basic medical insu rance that covers my participation. Therefore, players should have a current, active, p ersonal injury insurance policy in force, which covers my participation. Under any condition, players are responsible for any uncovered by the camp ́s basic insurance medical expenses aris ing from their participation, both in practices and games and while travelling to and fro m these events. Players have the right and responsibility to inspect the equipment and facilit ies prior to events and, if they believe that anything may be unsafe, they will advise the coach or supervisor of the condition and may refuse to participate. Participation assumes consen t.
Players, their parents and/or legal tutors hereby release, waive liability, discharge, hold harmless, indemnify, and covenant not to sue, La Li ga Futbol Camps, Inc. their associated directors, administrators, officers, managers, empl oyees, coaches, trainers, volunteers, sponsors and advertisers, and other agents, estates or executors, from any and all liability incurred in the conduct of, and my participation in , their soccer programs and the use of their facility. This includes owners, lessors, and lessee s of premises, municipalities, government agencies, successors, heirs, and assigns. Players, their parents and/or legal tutors have com pletely read this document and fully understand its contents. They acknowledge that they have given up substantial rights by accepting this document and that they do so volunta rily.
My La Liga Futbol Camps, Inc. Waiver of Liability a nd Release Form This form must be completed for each participant and, if under age 18 Must be signed by the player’s parent or legal guardian. Parent and natural guardians or legal guardians of the participants, they hereby agree to the foregoing Waiver of Liability and Release for, and on behalf of, the participant that they are responsible of. Parent and natural guardians or leg al guardians of the participants hereby bind themselfs, the minor, and all other assigns to the terms of the Waiver of Liability and Release. I represent and certify that they have the legal capa city and the authority to act for, and on behalf of, the minor in the execution of this Waive r of Liability and Release.
